Warfarin blocks vitamin K-dependent clotting factors (II, VII, IX and X), so vitamin K reverses it, over hours. In major bleeding, give IV vitamin K together with prothrombin complex concentrate, or fresh frozen plasma where that is not available, for faster reversal.
