Treating organophosphate poisoning
Pharmacology · exam-style practice question with explanation
A farmer has pinpoint pupils, excess salivation, sweating, bronchorrhoea and bradycardia after spraying pesticide. Which drug is given first?
- A. Atropine
- B. Adrenaline
- C. Naloxone
- D. Physostigmine
Show the answer and explanation
Answer: A. Atropine
These are cholinergic (muscarinic) features of organophosphate poisoning, which inhibits acetylcholinesterase. Give atropine in escalating doses until the chest is clear and secretions dry. Pralidoxime may be added to reactivate the enzyme. Remove contaminated clothing and wash the skin.
Key point: Organophosphates: atropine until secretions dry.
